Scientists Discover How a Sleeping Cancer Awakens and Metastasizes

A new study reveals that cancers in remission are capable of reawakening again. Researchers from the Cold Spring Harbor Laboratory (CSHL) demonstrated how sleeping cancers awaken and metastasize. Based on this knowledge, they developed a treatment plan that can prevent dormant cancer cells from awakening again.

The research was carried out in lab mice with hopes of being tested on humans soon.

According to the researchers, a patient successfully treated and cured of cancer can still become afflicted with the disease again. This is largely because dormant, non-dividing cancer cells that previously detached from an original tumor may move elsewhere to exist in other parts of the body. Once awakened, these dormant cancer cells can multiply and develop into metastatic tumors.

Cancers Become Most Fatal When They Spread Fast To Other Parts of the Body

Metastasis is the spreading of cancer disease from one part of the body to others.

The CSHL team together with scientists from the Dana Farber Cancer Institute and University of California, Davis, found triggers that could reawaken dormant cancer cells. They discovered this while studying metastasis to the lungs.

They found that inflammation of the lungs – caused by tobacco smoking and other factors – can make passive breast and prostate cancer cells which had earlier moved to the lungs to wake up and begin to multiply. The now active cells can grow all over the lungs.

Most common cancers become highly lethal when they metastasize or spread to become active in other parts of the body.

The scientists were however able to block the signals that awakened the dormant cancer cells, demonstrating that cancer progression could be arrested or reversed.

Led by CSHL Associate Professor Mikala Egeblad, the team found that neutrophils awaken surrounding passive cancer cells with lung inflammation caused by tobacco smoke or endotoxin. Neutrophils are common white blood cells, and endotoxin is a bacteria component.

Neutrophils destroy bacteria and yeast among other pathogens that invade our bodies. One of the ways they do this is to push out the DNA of the invaders outside of their cell membranes. Armed with toxic enzymes, the expelled DNA creates net-like traps to vanquish pathogens. The net-like trap is known as neutrophil extracellular traps or NETs.

NETs form around dormant cancer cells of patients having prolonged lung inflammation. NETs contains two enzymes known as neutrophil elastase (NE) and matrix metalloproteinase 9 (MMP9). Both NE and MMP9 interact with laminin, a tissue containing protein. NE goes ahead to cut into laminin proteins and MMP9 also does the same. This changes the protein’s shape, exposing a new surface, called an epitope.

When passive cancer cells sense the presence of epitope nearby, they are triggered to begin reawakening and then multiplying. As soon as dormant cancer cells recognize new shapes in the laminin, they are roused from sleep and begin to metastasize.

To prevent dormant cancer cells from rousing, the research team created an antibody that blocks the epitome exposed on the laminin proteins. The scientists have also begun to optimize the antibody to interfere with the activity of NETs, effectively keeping cancer in remission in rats. The researchers hope to begin conducting trials of this study on people soon.
